Position Summary Intake Coordinator LPN:

The Intake Coordinator LPN reports to the Administrator and serves as a resource person for outside referral sources and Agency staff. The Intake Coordinator is readily available to advise referral sources and members of Agency staff. The Intake Coordinator has primary responsibility for processing referrals, acts to resolve problems, and obtains adequate information from all referral sources. Provides daily oversight and coordination of intake activities for the Agency and oversees the Intake Specialist(s).

Required Experience for Intake Coordinator LPN:

  • MUST HAVE PREVIOUS HOME HEALTH INTAKE EXPERIENCE.
  • Currently LPN in the State of Michigan is preferred.
  • Minimum of 2 years of nursing experience, home care setting is required.
  • Excellent communications skills.
  • Compliant with accepted professional standards and practices.
  • Self-directed with the ability to work with little supervision.
  • Ability to read and interpret clinical documents, such as discharge plans, physician progress notes, and orders.

Accountability for Intake Coordinator LPN:

  • Accountable for identifying comprehensive needs and recommends appropriate changes in staffing, budgets, policies, or programs to meet those needs.
  • Demonstrated leadership qualities.
  • Adheres to privacy and security standards provided by all federal, state, and local authorities of the Health Insurance Portability and Accountability Act (HIPPA) and other applicable laws.
  • Promotes a positive and effective system of communication throughout the Agency.
  • Maintains and enhances professional expertise through education/organizational activities.
  • Participates in short- and long-range planning based on Agency goals and objectives.
  • Directs activities and evaluates effectiveness in intake.

Responsibility for Intake Coordinator LPN:

  • Oversee departmental staff (i.e. Intake Specialist).
  • Coordinate referrals and assist with case assignment.
  • Serve as a resource to case managers and physicians concerning home health, availability, and admission procedures.
  • Ensure appropriate triage and decision making for home health care patients with acute medical problems.
  • Reports cases of possible abuse, neglect, fraud, noncompliance, and exploitation to the Compliance Officer, Administrator, or Designee immediately.
  • Assist in the development of policies and procedures related to referral, intake, and data entry.
  • Receive incoming calls from referral sources and transcribe the information to process the referral and the admission of the patient.
  • Audit referrals prior to assignment, to assure clinical information received, supports reimbursement and outcome targets for the Agency.
  • Assist with the orientation of new personnel, as required.
  • Assist in all administrative areas of the department, as needed, including insurance authorization and authorization tracking.
  • Compliance with all applicable laws, regulatory requirements, standards of practice, accreditation standards and policies and procedures.
  • Assure all adequate information for referrals is completed.
  • Maintains departmental computerized tracking for specific data base reporting.
  • Provides education to referral sources and agency staff related to necessary items in processing referrals.
  • Takes part in the Agency’s Compliance and QAPI plans.
  • Contacts Administrator to inform him/her of alternate schedule (i.e. vacation, sick, or personal days).
  • Maintains OSHA and Infection Control per policy.
  • Performs other duties as assigned.
